General Office Policies
|
|
|
|
The Office will not schedule appointments for those new patients who are cash pay and/or those whose
primary insurance is Medicaid unless they have a referral from their primary care physician.
|
|
|
|
Co-Insurance (co-pay) payments can be made using Mastercard™ or Visa™ credit cards, via personal or bank check, or using cash.
|
|
|
|
Co-pays on all insurance-related appointments are expected at the time of the appointment.
All co-pays must be paid at the “specialist” rate.
|
|
|
|
Patient outstanding balances for previous appointments
or treatments must be paid in full prior to making any
additional appointments. Accounts on Patients who fail to
pay their balance may be turned over to acollection agency
for payment, and/or the patient may be discharged, at the
discretion of the physician.
|
|
|
|
As a courtesy to our allergy patients, we ask that you
do not wear cologne or perfume while in the Office.
There will be no allergy injections given when the physician
is not in the office.
|
|
|
Prescriptions and Drug Information
|
All new, expired and refill prescription requests require an Office appointment.
In rare cases, prescription refill requests can be made
via telephone, by calling the Office. Please note that it can take 24-48 hours to fulfill requests as
prescriptions can only be called in on Tuesday, Wednesday and Friday.
All Class II narcotic prescriptions require a monthly appointment with the doctor. These Class II narcotics include such drugs
as Percocet, Lorcet, MS Contin, duragesic patch, and Morphine Sulfate.
Early refills will not be given for controlled medications such as Hydrocodone, Percocet, MS Contin, and Methadone.
It very important that patients take full responsibility of their controlled medications. Medications may not be replaced if they
are lost, destroyed, left on an airplane, etc. If your controlled medication has been stolen and you complete a police report regarding the theft,
an exception may be made. Repeated conditions will not be accepted regardless of reason.

Health Insurance Information
|
Patients with a HMO Insurance plan are responsible for their own referrals. The Office will not schedule appointments for HMO
Insurance patients without a valid referral from their Primary Care Physician (PCP). As Dr. Tsai is a specialist physician, he will not act as PCP under any circumstance.
Our office will send a follow up report from your visit to your primary care physician along with your next
appointment date. The office will not call your primary physician to request for the referral on your appointment date. Your appointment will be rescheduled if there is no referral.

Appointment Limitations
|
The Office will not accept appointments for patients involved in auto accidents, Workman’s Compensation, or Functional Capacity
evaluations for patients involved solely in applications for Social Security disability benefits.
No appointment is needed for Methotrexate (MTX), Enbrel, Humira or allergy injections, provided that patients come at specified times, available by contacting the Office.

Cancellations and Rescheduling
|
Those patients that miss office appointments and those that do not cancel or reschedule at least two business days prior to that time, will then be subject to the no show policy below.
Patients contacting the office prior to the two business day deadline will be offered the opportunity to either reschedule or cancel their
appointments, and if not possible at that time, asked to contact the office at a later stage to attempt to reschedule.

No-Shows
|
Patients will be considered a, “no-show” at the close of business on the day of their missed appointment.
Patients are given an, “allowance” of no-show appointment Opportunities in any rolling six month time period.
Please understand, If the patient do not cancel or show up for their appointment, they are keeping another patient from seeing the physician.
If the patient is considered a “no-show” 2 times in 1 year, the office will not give the patient an appointment. The patient will be
encouraged to call 1 week before their medication runs out so they could be placed in the schedule.
